Ipamorelin (development code NNC 26-0161) is a synthetic pentapeptide growth hormone secretagogue with the sequence Aib-His-D-2-Nal-D-Phe-Lys-NH2, in which Aib denotes alpha-aminoisobutyric acid and D-2-Nal denotes 3-(2-naphthyl)-D-alanine. Its molecular formula is C38H49N9O5, corresponding to an average molecular weight of 711.9 g/mol, and it is registered under CAS 170851-70-4. Two of the five residues are non-proteinogenic and two carry the D-configuration; together with the C-terminal carboxamide, these features confer marked resistance to exopeptidase hydrolysis and distinguish the molecule structurally from the far larger growth hormone-releasing hormone analogues, with which it shares no sequence homology. Ipamorelin is typically isolated as an acetate or trifluoroacetate salt and presents as a white to off-white lyophilized powder that is readily soluble in water and in dimethyl sulfoxide.

Ipamorelin has been characterized in vitro as a selective agonist at the growth hormone secretagogue receptor 1a (ghrelin receptor), with receptor selectivity and pituitary secretory profiles established in isolated rat pituitary cell preparations and in rodent tissue systems. It has been studied in rat models in which longitudinal skeletal endpoints, bone mineral content in adult females, and bone-formation indices under glucocorticoid exposure were the measured variables. Gastrointestinal transit parameters have been examined in a rodent model of postoperative ileus, and body-mass and emetic endpoints have been assessed in ferrets exposed to cisplatin. Nociceptive thresholds have been investigated in rodent visceral and somatic preparations, and hypothalamic-pituitary-gonadal axis parameters have been examined in a cichlid fish model. Biophysical work has characterized its secondary structure, thermal stability and interaction with model membrane systems.
